Chronic Disease and Injury Prevention

The Position: Under the general supervision of the Chronic Disease and Injury Prevention (CDIP) Program Manager, this position, in the classification of Public Health Educator, emphasizes health promotion and disease prevention for entire populations. This position will support SNAP-Education and other program initiatives as assigned, providing community resource education in support of the program. The incumbent will work within a multidisciplinary team to achieve the long-term goal of preventing chronic disease and reducing unintentional injury. The incumbent will support and facilitate community focused initiatives to address population-based health priorities and maintain partnerships that produce broad-based health collaboration. The incumber will support the design of evidence-based strategies and evaluate efforts to determine community impacts and outcomes. 

Hours: 40 hours per week – 100% FTE. This is an in-office assignment. The incumbent will report to the district offices located in the Norm Dicks Government Center in downtown Bremerton, Monday through Friday, 8:00 a.m. until 4:30 p.m.

Please note: The incumbent is expected to be available during the agency’s hours of operations and some evening and weekend work may be required for public meetings and/or other associated events. A limited hybrid telework arrangement may be established as mutually beneficial after adequate onboarding and sustained successful performance, generally after at least three months. 
 
Status: Regular status, full-time position.

Essential Functions:

  • Assists in the promotion of inclusive strategies to engage multiple sectors of the community in the area of assignment.
  • Assists team members to support community agencies and other stakeholders with opportunities to engage individuals to promote healthy behaviors and make enhancements to environments that improve community health.
  • Assists in the facilitation of stakeholder groups.
  • Assists in the facilitation and achievement of grant deliverables by focusing on policies, systems, and environmental changes.
  • Adis existing community groups, coalitions, boards, and service providers to increase their capacity to develop community health programs, policy initiatives, and community-level strategies.
  • Participates in all required meetings, trainings, and conference calls as required.
  • Participates in staff and team meetings and committees; prepares reports as requested.
  • Maintain knowledge of policies, procedures, and grant requirements; performs duties in accordance with applicable regulatory and grant requirements, regulations, and accreditation standards.
  • Responsible for various aspects of access to health program operation, grant management and review, budgeting, reporting, and evaluation. 
  • Actively assists and participates to develop and distribute communication documents created for the funder, stakeholder, and/or community members, highlighting the success of our work.
  • Prepares public health information, letters, memos, forms, reports, and other documents; operates computers utilizing a variety of software programs, including databased and word processing applications, to produce documents with clearly organized thought using proper sentence construction, punctuation, and grammar.
  • Accurately completes and submits electronic timecard on a weekly basis as required.
  • Works both independently and within a collaborative team-oriented environment; contributes openly, respectfully disagrees, understands the ideas of others, listens well and works for consensus.

  • Reports for scheduled work with regular, reliable and punctual attendance.  
  • Performs other duties as assigned.

Education & Experience: 

  • A bachelor's degree preferably in health education or equivalent; and
  • At least one year experience in health education, nutrition, community organization and/or health education or an equivalent combination of experience and education; or
  • Any equivalent combination of education and lived experience may be qualifying, provided the individual's background demonstrates the knowledge, skills, and abilities required. 

The ability to comfortably lead groups and talk to community members is required. Organizational skills and the ability to work with a variety of community partners is a plus. Comfort in working with both youth and adults is preferred.  
 
Degrees must be from appropriately accredited institutions.

Licenses, Certificates, & Other Requirements:

  • Performance of job duties required driving on a regular basis, a valid Washington State driver's license, the use of the incumbent's personal motor vehicle when a District fleet vehicle is not available for use, and proof of appropriate auto insurance.
